I'm glad River was turned into an actual character, but she still annoyed me. *g*

I think older Timelords would know the Doctor's real name (and could have looked it up in the Matrix or whatever), but they respected his wish to be called the Doctor, just as they did with the Master. :) The Doctor has never been called anything but the Doctor, so I'm assuming that he never told Romana. ;D
River being Romana was my first guess, but then the Doctor would have been able to feel her in his mind and all that, so...
Thoooooough, he did say something about there only being a time he could say his name. I donno, is it just a ceremonial thing after he picked a new one?
Can he only say it on Gallifrey? At a wedding? -shrugs-
I want Gallifrey back! I have a bit of a hatred of the Timelords for what they did to Two, Jamie and Zoe, but I love their silly costumes, and I do enjoy mocking them.
